Prospective Riders
The completion and return of all pertinent forms will place the prospective rider on the Alisons' Whisp
ers Wait List (Rider Application Form and Medical Physician Form).
The next step in the process is to set up an Individual Assessment with an Alisons' Whispers staff member.
Prospective riders are placed into the Alisons' Whispers programs based upon information obtained from the review of forms, the Individual Assessment, and scheduling needs/availability. Please note that the Medical Physician form must be completed, signed and dated by the prospective rider's doctor in order to be valid for our records.
